Starting January 1, 2012, tobacco products (use of cigarettes, cigars, pipes, e:cigarettes and smokeless tobacco products) will not be allowed to be consumed on Orange County Health Department property including buildings (interior and exterior), parking to comply with the Tobacco Free policy on any facility, campus, or building that is owned or leased by, or on behalf of, OCHD including: buildings (interior and exterior), walkways, parking lots, public and private vehicles parked in the lots, and any public sidewalks that fall within the OCHD facility boundaries. Any person who violates the Tobacco Free policy may be subject to disciplinary action.

The Human Services Program Consultant:I/ Linkage Coordinator (LC) is a professional HIV/AIDS prevention position within the Area 7 HIV/AIDS Program responsible for linking newly diagnosed or returning to care HIV + positive individuals referred by public and/or private HIV testing programs into the HIV/AIDS system of care within Area7 utilizing Test and Treat. The incumbent will meet face:to:face with all individuals referred to him/her and link them to medical care within thirty (30) days of their HIV diagnosis. This position reports to the Operations and Management Consultant:II/ Early Intervention Consultant (EIC).

:Collaborate and coordinate with all HIV counseling and testing staff at County Health Departments (CHD), public and private medical providers, HIV/AIDS service organizations (ASO), State and County correctional staff and other community HIV testing sites to link HIV+ positive individuals into the HIV/AIDS system of care throughout Brevard, Orange, Osceola, and Seminole Counties. Complete initial Ryan White Patient Care Eligibility Application when necessary and set up initial appointment(s) for access into the medical care system for these individuals. Follow up with the provider and individual to confirm that the individual kept their set appointment.

:Enter all testing and linkage services into the Health Management System (HMS), the State Ryan White CAREWare system, the STD PRISM system and any and all other reporting systems as needed on a daily basis. Explore and disseminate information on existing community services that would assist HIV+ positive individuals with the practical aspects of adherence such as transportation, obtaining needed medications, receiving Medicaid or private insurance, etc. Provide HIV education and information to HIV + positive individuals as needed.

Coordinate with all HIV counseling and testing staff at CHDs public and private counseling, testing and medical treatment staff to provide linkage information for quarterly reports to the HIV/AIDS Section. Compile and submit monthly reports to supervisor for review by the 5th business day of each month. This report should include, at a minimum, the number of individuals tested and linked to medical care, other social services, i.e., food, housing, transportation, etc. Submit approved monthly reports to the Prevention Program at the HIV/AIDS Section by the 15th of each month. Perform HIV/AIDS Counseling/Testing/Linkage (CTL) duties as needed/assigned.
